Your first appointment will take approximately 1 hour. The length of time involves initial paperwork, doctor’s consultation, evaluation, possible x-rays, and treatment. Your benefits will be verified at the time of your first appointment. Please be sure to bring along your insurance card and driver’s license to this appointment. There are several forms available to print off and either bring with you to your appointment or email to us in advance of your appointment. To view these forms, please go to the "Patient Resources" tab of our website. You will be completing the following forms:
- New Patient Registration
- Patient Health History
- Pain Rating Measure
- Appropriate Pain Measure
Charges will vary depending on services provided. There is no charge to discuss your health problem with a Doctor.
Yes. We make every effort to see you the same day you call.
The Doctor does not always take x-rays and will tell you if they are necessary. Often time he does not. There may be an additional charge if x-rays are required.
At the time of your first appointment, we will verify your chiropractic benefits. All plans are different and it is necessary for us to talk directly to your insurance company before we answer this question for you. We will try our best to provide this information to you before you are seen for your appointment that you are aware of what is and is not covered on your plan.
